My Home Nurses is seeking a compassionate and dedicated Licensed Physical Therapist (PT) to join our home health team on a PRN, Full-time, per-visit basis. Responsibilities
- Perform comprehensive patient evaluations and assessments.
- Develop and implement individualized treatment plans.
- Provide skilled physical therapy interventions and patient education.
- Monitor patient progress and modify treatment plans as needed.
- Collaborate with physicians and the interdisciplinary care team.
- Complete accurate and timely clinical documentation.
- Maintain compliance with Medicare, state, and agency requirements.
Qualifications
- Active Illinois Physical Therapist (PT) license, or eligibility to obtain Illinois licensure.
- Graduate of an accredited Physical Therapy program.
- Current CPR/BLS certification.
- Strong clinical, communication, and documentation skills.
- Ability to work independently while managing a flexible caseload.
- Reliable transportation and willingness to travel throughout the Chicago service area.
- Home health experience is preferred but not required.
- New graduates are encouraged to apply.
