Job Overview:
We are seeking dedicated Licensed Vocational Nurses to join our healthcare team. The ideal candidates will provide one-on-one patient care for the client in San Mateo under the direction of physician and registered nurses, ensuring the highest quality of care for this client.
Duties include, but not limited to:
- Have a reliable vehicle and a valid driver's license as driving is necessary in this position
- Administer treatment at 9:30 pm every night as prescribed by healthcare provider
- Monitor and assess patient condition in their home, reporting changes to the healthcare team
- Collaborate with interdisciplinary teams to develop and implement care plan
- Educate patient and family on health maintenance and disease prevention
- Perform various nursing procedures
- Job is located in San Mateo County, California
Qualifications:
- Must hold a current Licensed Vocational Nurse (LVN) license in the state of CA
- Proficient in medical terminology
- Experience in home health care and patient education is a plus
- Ability to perform triage assessments accurately
- Strong skills in patient care coordination
