The Intensive In-Home QP works collaboratively with families to implement person-centered plans, deliver evidence-based interventions, and support positive outcomes.
- Develop and monitor person-centered plans for youth and families.

Provide supportive counseling and crisis intervention.

Assist families in accessing educational, vocational, and housing resources.

Document services accurately in the information system.

Participate in team meetings and clinical supervision.
- Required: Bachelor’s degree in Human Services with min of two years of experience.
