Multiple Locations: San Diego County
Per Diem Position:
- Pay is based on per visit completed
- Admission/Initial Assessment Visit: per 150 visit (1.5 - 2 Hrs)
- Recertification Visit (Reassessment): per 75 visit (1 hour) every 90 days
- Skilled Nursing Visit (SN Visit): per 65 visit (45 min.)
Responsibilities:
- Conduct comprehensive patient assessments to evaluate symptoms, needs, and care goals, utilizing your knowledge of physiology and medical terminology.
- Develop and implement individualized hospice care plans in collaboration with interdisciplinary teams, including physicians, social workers, and therapists.
- Manage symptom control and medication administration to ensure patient comfort, including pain management and infectious disease care protocols.
- Monitor vital signs and perform diagnostic evaluations to track patient progress and adjust care strategies accordingly.
- Provide education and emotional support to patients and families regarding end-of-life processes, discharge planning, and post-acute care options.
- Document all patient interactions accurately in medical records while adhering to HIPAA regulations for patient privacy.
- Coordinate with case management and utilization management teams to optimize patient outcomes and facilitate smooth transitions between inpatient, outpatient, or home settings.
Qualifications
- Valid RN license with current registration; additional certification in hospice or palliative care is a plus.
- Critical care experience preferred, with familiarity in ICU, emergency medicine, or hospital medicine settings.
- Experience working with diverse populations including geriatrics, pediatrics (PICU), behavioral health patients, or those with complex chronic conditions.
- Knowledge of hospital experience procedures such as intake assessments, discharge planning, medical records management, and vital signs monitoring.
- Strong understanding of medical-surgical nursing practices along with expertise in diagnostic evaluation and case management.
- Ability to work independently while maintaining excellent patient service skills; flexibility for per diem scheduling is essential.
