Job Overview
We are seeking a compassionate and dedicated Registered Nurse for our Home Health division. Home Health experience is REQUIRED. In this role, you will provide high-quality nursing care to patients in their homes, ensuring their comfort and well-being. You will collaborate with healthcare teams to develop and implement individualized care plans, educate patients and families, and monitor patient progress.
Duties
- Conduct comprehensive assessments of patients' health status in a home setting.
- Develop, implement, and evaluate individualized care plans based on patient needs.
- Provide direct nursing care, including administering medications and treatments as prescribed.
- Educate patients and families on health management, disease prevention, and self-care techniques.
- Collaborate with interdisciplinary teams to coordinate care and ensure optimal patient outcomes.
- Maintain accurate and up-to-date patient records, documenting all assessments, interventions, and outcomes.
- Perform triage duties as necessary to prioritize patient needs effectively.
- Ensure compliance with healthcare regulations and standards of practice.
Skills
- Must be a graduate from an accredited school of Nursing
- Must have Home Health experience
- Must be licensed in California as a Registered Nurse
- Wellsky Kinnser experience preferred

Starting Pay:
$60 per visit with D/C summary
$65 per evaluation/start of care visit
