Responsibilities
- Perform comprehensive patient assessments, including vital signs, physical examinations, and physiological evaluations to inform care plans.
- Administer medications and injections accurately, including IV infusions and sterile procedures, following all safety protocols.
- Document all patient interactions meticulously within electronic health record (EHR) systems such as Kinnser, ensuring compliance with HIPAA regulations.
- Provide specialized care for individuals with complex conditions such as Alzheimer's disease, dementia, or those requiring memory or behavioral health management.
- Assist with diagnostic evaluations including specimen collection, processing, and interpretation of results using medical imaging and laboratory techniques.
- Support discharge planning and case management efforts to ensure seamless transitions across care settings.
- Engage in patient education and health coaching to promote wellness and adherence to treatment plans.
Qualifications
- Valid RN or LVN license in good standing; current CPR/BLS certification required.
- Proven experience working in Home Health or Hospice.
- Familiarity with EMR/EHR systems including Kinnser.
- Demonstrated knowledge of medical terminology, anatomy, physiology, and pharmacology relevant to acute and chronic patient care.
- Experience with specialized nursing procedures such as IV insertion, catheterization, suturing, tube feeding, and sterile techniques.
- Ability to perform physical assessments and diagnostic evaluations accurately; experience with ventilator management and airway support is advantageous.
- Strong critical thinking skills with the ability to triage effectively in urgent situations; familiarity with infectious disease control and infection prevention protocols.
- Compassionate approach to working with diverse populations including pediatrics, geriatrics, individuals with disabilities or developmental disorders such as autism or Alzheimer's disease.

Pay: $55.00 - $125.00 per hour
