Reports To: Administrator, Director of Nursing (DON), Quality Assurance Director
Tropical Health Hospice is seeking an experienced and compassionate Registered Nurse (RN) to oversee and coordinate patient care services for hospice patients receiving care in their homes. The RN is responsible for ensuring quality patient care through assessment, care planning, clinical supervision, evaluation, and coordination of the interdisciplinary care team.
Active Registered Nurse (RN) license in the State of Georgia.
Minimum of 3 years of nursing experience required.
Hospice experience preferred.
Previous supervisory or management experience preferred.
Strong clinical assessment and leadership skills.
Excellent verbal and written communication abilities.
Ability to work independently and collaboratively within an interdisciplinary team.
Ability to respond to the needs of terminally ill patients and their families.
Flexible schedule and participation in on-call rotation required.
Coordinate and supervise the delivery of hospice services for patients and families.
Ensure clinical services comply with hospice regulations, agency policies, and standards of care.
Maintain accurate and compliant clinical documentation.
Assist with patient admissions, certifications, recertifications, and discharges.
Provide clinical guidance and support to nurses and interdisciplinary team members.
Participate in quality assurance and performance improvement activities.
Assist with staff orientation, training, supervision, and performance evaluations.
Monitor clinician caseloads and scheduling.
Coordinate care with physicians, the Medical Director, and interdisciplinary team members.
Participate in administrative and nursing on-call responsibilities.
Investigate and report patient concerns, grievances, incidents, and safety issues.
Ensure compliance with federal, state, and accrediting agency requirements.
Support community outreach and hospice marketing initiatives.
Perform other duties as assigned by leadership.
Ability to sit, stand, walk, bend, kneel, reach, and lift up to 50 pounds occasionally.
Ability to travel to patient homes and other care settings as required.
Must possess visual, hearing, and communication abilities necessary to perform nursing duties.
May be exposed to infectious diseases, odors, disruptive patient behavior, and other healthcare-related environmental conditions.
