Job Overview
We are seeking a dedicated therapy professional to provide high-quality care in a skilled nursing and rehabilitation setting. This role focuses on helping patients improve function, regain independence, and enhance overall quality of life.
Key Responsibilities
Evaluate and develop individualized care plans
Provide hands-on therapy and rehabilitation services
Monitor patient progress and adjust treatment plans
Collaborate with interdisciplinary teams
Educate patients and families
Maintain accurate documentation
Requirements
Active state license (PT, OT, or relevant therapy discipline)
Experience in skilled nursing or rehab preferred
Strong communication and patient care skills
