Job Summary: The Youth and Recovery Services Coordinator will serve as the key for the TREE program, guiding participants through the full continuum of services—from initial outreach and screening to enrollment, assessment, service planning, implementation, and successful discharge. The primary responsibility of this role is to ensure consistent program enrollment and engagement of the target population—young adults aged 18 to 25—through proactive street outreach and strong community partnerships.
Working closely with the Site Director, this position coordinates daily outreach activities and provides leadership and support to the TREE team. The Youth and Recovery Services Coordinator collaborates with the youth Substance Use Counselor, Sr. Housing Specialist, Employment Specialist, and Program Assistant to ensure that all program interventions, recovery support services, and client engagements are effectively implemented, monitored, and documented in accordance with program requirements and data reporting systems. This position requires a highly organized and motivated professional with strong leadership and communication skills. The ideal candidate demonstrates the ability to troubleshoot challenges, anticipate trends affecting the target population, and provide detailed and timely reports to the program funder, the Substance Abuse and Mental Health Services Administration (SAMHSA).
Education and Work Experience Requirements:
- Minimum: Master’s of Social Work or similar degree required.
- Minimum 2 years of experience in program coordination.
- Minimum 2 years of experience coordinating and conducting field-based community outreach in community venues, including but not limited to shelters, health fairs, and on the street, to reach high-risk populations
- Minimum 2 years of experience working with the young adult population, individuals using substances, and/or those who are unstably housed
- Minimum 1 year experience administering rapid HIV, HCV, and STI tests, providing counseling on risk reduction and interpreting results. Experienced phlebotomist, Comfort with conducting blood-based rapid tests (finger-stick)
- Flexibility to work evenings (at least 2 per week) and weekends (at least 3 per month).
- Strong experience with AIRS, electronic health records (EHR), and other databases used to report program deliverables to funders.
Certification/Licensure: Minimum NYS LMHC/LMSW - Limited Permit eligibility required LMHC/LMSW/LCAT/LMFT preferred. CRPA -Y or CASAC T and above a plus
Language Skills: Bilingual: Haitian Creole/English or English/ Spanish preferred

Description of Duties and Responsibilities:
TREE Program Coordination
- Coordinate and participate in weekly outreach activities with the TREE team, including internal departments and community partners such as drop-in centers, foster care programs, and street outreach programs.
- Maintain a consistent caseload and develop referral partnerships.
- Develop and present a weekly outreach plan and results report during team meetings.
- Plan, coordinate, and execute outreach events including presentations, community tabling, and agency outreach initiatives to increase program enrollment.
- Manage TREE Recovery Bed program, including, but not limited to, client intake, discharge, follow-up, and client communications.
- Build and maintain strong relationships with internal departments and external partners to ensure effective referrals and service delivery.
- Collaborate with the Site Director and the Marketing and Communications team to develop outreach materials, including letters, emails, and event flyers.
Program Services
- Work toward achieving the program’s key performance indicators.
- Complete all required SUPRT assessments at baseline, 3-month, 6-month, and discharge intervals.
- Conduct substance use and related assessments within the first month of enrollment, including: Housing Assessment, MSSI and SUPRT Tools
- Submit completed SUPRT Tools to the Client Service Representative for entry into SPARS within one business day.
- Provide smoking-cessation interventions to program participants.
- Facilitate individual and group counseling using risk reduction, person-centered, and trauma-informed approaches.
- Develop individualized service plans and conduct monthly reviews to ensure coordinated care.
- Establish and facilitate the Young Adult Advisory Group, meeting monthly with a minimum of five participants.
- Serve as a resource on young adult substance use for staff across the department and agency.
- Utilize internal and community resources creatively to enhance client engagement and service delivery.
- Provide housing guidance and referrals to the Senior Housing Specialist when appropriate.
- Complete psychosocial assessments in the CAPS system.
- Coordinate mental health referrals for psychological evaluations when needed.
Documentation and Reporting
- Submit all required documentation to the Program Assistant within one business day, including: Baseline, 3-month, 6-month
- Discharge SUPRT Tools
- Work collaboratively with the Program Assistant to ensure accurate and timely submission to SPARS, the Substance Abuse and Mental Health Services Administration reporting system
- Document all client encounters, interventions, and assessments in eCR within two business days.
- Complete and submit Community Level Intervention (CLI) forms, outreach summaries, and sign-in sheets within one business day.
- Update the program performance tracking document (strategic plan) weekly, including: Outreach tracking, Referral tracking, Reassessment tracking
- Ensure housing referrals and recovery apartment referrals are completed and submitted to program leadership for review and approval.
Team and Operational Responsibilities
- Participate in weekly team meetings.
- Record and distribute meeting minutes.
- Attend weekly individual supervision sessions with the Site Director.
- Co-facilitate monthly group meetings and other assigned meetings.
- Prepare TREE program reports and conduct data analysis.
- Conduct HIV and Hepatitis C testing.
- Support site operations, including but not limited to: Assisting with Flatbush Gardens events and community engagement, supporting Article 11 program tenant engagement, and managing visitor logs
- Participating in front desk coverage based on team rotation
Skills, Knowledge, and Abilities Required:
- High Proficiency in Microsoft Office Suite
- Strong Knowledge of: Motivational Interviewing, Stage-wise treatment, IDDT, WRAP, Harm Reduction practices
- Strong ability to prioritize tasks, follow policies and procedures, and meet deadlines
- Experience delivering services through a trauma-informed care framework
- Strong verbal de-escalation and crisis management skills
- Ability to provide culturally competent services and demonstrate respect for diverse communities
- Demonstrated application of: Harm reduction principles, Motivational interviewing, Stages of change model
- Strong knowledge of resources available in NYC for youth and families affected by substance use
