We are seeking a compassionate and skilled RN Case Manager to provide exceptional home health care throughout the Monroe and surrounding service area as we continue expanding our patient census in this growing territory.

This role is ideal for an RN who values autonomy, strong clinical judgment, and the opportunity to grow alongside an expanding patient census.

Conduct comprehensive nursing assessments

Develop individualized plans of care

Deliver skilled nursing interventions in the home

Educate patients and families on condition management and medications

Coordinate with physicians, therapists, and interdisciplinary team members

Complete accurate and timely documentation in WellSky / Kinnser

Utilize Home Health Notify for real-time communication and updates

Ensure compliance with state, federal, CHAP, and Joint Commission standards

Graduated from an accredited school of nursing

Hold an active Michigan RN license

Have at least 2 years of nursing experience

Home health experience preferred

Are compassionate, organized, and resourceful

Have strong assessment and communication skills

Have reliable transportation, valid auto insurance, and a driver’s license
