Job Summary: Building a relationship with the residents, families, visitors, and other members of the health care team. We are a resident-centered Health Center, maintaining consistent assignments and wanting each person to be familiar with their residents’ needs, personal background, and preferences. Responsible for performing resident care tasks under the direction and supervision of the charge nurse.
Responsibilities:
Will complete the following duties as the resident prefers:
- Baths assigned to residents either by sponge bath, showering, or tub baths (including whirlpool baths).
- Assists as needed in cleaning teeth, shampooing hair, combing hair, cleaning, and clipping the resident’s nails, and applying makeup.
- Assists residents in dressing appropriately and encourages them to wear clothing suitable for the season.
- Make resident beds and change linens as needed, clean resident rooms and closets, pick up unnecessary articles, and arrange the room neatly.
- Assist ill and/or dependent residents with meal service.
- Answers call light promptly and is courteous to residents.
- Assists resident with bedpan, urinal, bedside commode, of to the bathroom as needed and/or directed.
- Measures vital signs (temperature, pulse, respiration, and blood pressure) as requested and reports any abnormal or unusual vital signs to the charge nurse.
- Measures intake and output on all residents for whom it has been ordered
- Empties and measures urine from an indwelling catheter or other urine collection device near the end of their shift and relays information to the charge nurse to record on I & O sheets
- Reports unusual color or odor of excreta (urine, drainage, stool, or sputum) before discarding it.
- Record the amount of voiding, bowel movements, and emesis on daily assignment sheets, and report abnormal findings to the charge nurse immediately.
- Assist in collecting specimens and performing diabetic urine testing as directed.
- Utilize precautionary measures to ensure residents' and own safety when transferring the resident to a wheelchair, lifting, turning, and /or moving the resident with the use of a gait belt as needed and following our non-lift policy.
- Help maintain the personal dignity of each resident by ensuring privacy and treating each resident with respect.
- Use direct observation as needed to focus on resident safety.
- Ensure that the call light, water, etc., are always within a resident’s reach.
- Shows empathy and understanding to residents, families, and visitors.
- Assists in admission, transfer, and discharge of residents.
- Helps with any other tasks that the resident or supervisor may request.
Restorative Aide Responsibilities
· Assist residents with walking, transferring, positioning, and mobility exercises. (only if 1 person assists)
· Help with range-of-motion (ROM) exercises as directed by the care plan.
· Assist residents with activities of daily living (ADLs)
· Encourage residents to do as much as they safely can for themselves.
· Follow individualized restorative nursing programs
· Help residents use walkers, wheelchairs, gait belts, and other approved equipment.
· Monitor residents during activities and report changes in condition, pain, weakness, falls, or other concerns to the nurse.
· Document services provided and residents' responses according to facility procedures.
· Maintain resident safety, dignity, privacy, and infection-control practices.
· Communicate with nurses, CNAs, therapists, and other members of the care team.
· Participate in activities designed to maintain strength, flexibility, independence, and functional ability.
Important: The exact duties and what you are legally permitted to perform can depend on your job title, training/certification, facility policies, and Oklahoma requirements. A Restorative Aide generally works under nursing/therapy direction rather than independently diagnosing or creating a rehabilitation plan.
Environment:
- Observes universal precautions
- Washes hands at the beginning of shift, between residents, before passing trays, and after assisting with elimination.
- Uses all equipment safely and reports equipment needing service or repair
- Maintain all service areas in clean and orderly condition and keep halls clear of equipment.
- Is aware of location and use of fire extinguisher and participates appropriately in fire drills and understands the disaster plan.
Other:
- Attend in-service education meetings regularly; a minimum of one hour per month.
- Follows the dress code for the nursing department, including the wearing of closed shoes with non-skid soles.
- Perform other duties as assigned.
Qualifications:
- Minimum Age: 16 years
- Grammar school education required; high school education preferred.
- Must be able to read, write, and speak English.
- Must have a current Oklahoma Certified Nurse Aide certificate, with no abuse notations.
- Must have a physical and mental capability to perform duties safely.
