Responsible for the overall administration, clinical planning, development, marketing and operations of the Rehab program within the facility. Manages and directs staff in the provision of program services to patients, residents and clients.
- Ensures that the quality and appropriateness of patient care meets or exceeds company and industry standards and ensures that all Rehab services comply with state and federal legal, regulatory, accreditation and reimbursement guidelines.
- Directs and assesses the scheduling of patients/residents for therapy services considering available treatment hours, staff resources and overall plan of care.
- Participates in team meetings to formulate treatment plans, discuss progress and discharge planning, attends family/care giver conferences as needed or appropriate.
- Reviews therapist documentation practices for clarity, conciseness and consistency with patient outcome goals and payer expectations.
- Monitors and manages efficient utilization of therapy services by discipline according to diagnostic and functional groupings.
- Maintains quality of life for entire population of facility by actively participating in team committees that address falls prevention, improving skin integrity, reducing restraint utilization, and maintaining ADL function and mobility.
- Uses Quality Measure reports as a source in guiding interdisciplinary team focus.
- Organizes the tracking and analysis of patient/resident outcome data; promotes an outcome-focused approach to care planning. Ensures that optimal level of rehab services is provided to meet clinical needs of patients/residents.
- Maintains a clinical caseload as needed. Under a physician’s order, provides direct treatment in accordance with an established plan of care. Treatment venue includes individual, supervised and group.
- Generates and analyzes financial, operational and productivity reports as required; contributes to the development of the annual operating and capital budget.
- Recommends the purchase of therapy equipment and supplies appropriate to the needs of the persons served within the facility by responding to and anticipating changes.
- Ensures that weekly and monthly statistics, patient outcome data, and billing logs are accurate, complete and reported in a timely manner.
- Demonstrates a good working knowledge of Medicare and other reimbursement regulators.
- Responsible for Human Resource issues of assigned staff. Maintains authority to provide remedial counseling, mentor and evaluate staff in accordance with company policies.
- Maintains sufficient staffing by responding to changes with impact staffing needs; ensure appropriate levels of professional and paraprofessional staff and maintains the clinical skill levels needed for the Program in accordance with budgetary and company guidelines.
- Ensures that there is a per diem (PRN) coverage plan; maintains an available per diem pool. Appropriately utilizes per diem staff in response to census changes and staff absences.
- Actively participates in the recruitment, hiring, and orientation of new staff. Provides ongoing development of staff by associating policies and procedures with the mission and values of the company.
- Supervises program staff in daily program operations and clinical activities; monitors staff efficiencies and routinely provides feedback. Continuously promotes interdisciplinary, clinical reasoning skills and best practices within rehabilitation.
- Attends and participates in corporate business and clinical meetings. Exhibits follow through with communicating new policies, programs, procedures, etc., to program staff.
- Attends business and clinical meetings of the facility, as a department director with other ancillary departments, including Admissions meeting and Medicare Conferences.
- Ensures ongoing communication with Facility Administration in order to keep them informed of key issues and program development opportunities.
- Participates in design and implementation of a facility-marketing plan in collaboration with facility administrators and other key personnel.
- Participates in education activities for referral sources and participates in pre-screening functions.
- Initiates the coordination of program development activities with the Administrator, Director of Nursing, Medical Director and other department directors for the purpose of introducing new and relevant services for the persons served and the community.
- In conjunction with other program staff, prepares and schedules regular in-service education programs for the Rehab Department and facility staff. Routinely conducts staff meetings. Promotes optimal communication among program staff members, facility staff, referral sources, physicians and patients.
- Appropriately accesses the resources of the company’s corporate offices for consultation and program development support. Seeks out external resources through conferences, workshops, etc., as necessary. Routinely shares personnel knowledge with staff, other managers and facility personnel.
- Demonstrates flexibility in adjusting to change, patient needs and customer expectations, meets company standards relating to deadlines and attendance.
- Establishes and maintains exemplary standards for department appearance and staff presentment; ensures compliance with company and facility policies and procedures.
- Responsible for all safety and risk management protocols and programs.
- Participates in direct and indirect patient care activities as warranted.
- Monitors and reports on the quality and effectiveness of Rehab services; monitors customer satisfaction and responds to complaints/concerns. Turns expressed complaints or concerns into solutions to existing problems in a timely fashion.
- Participates in quality improvement activities, which reduce cost and increase efficiency through improved systems and processes.
- Responsible for service and operational excellence of all assigned activities to ensure the delivery of quality services and/or outcomes required to meet or exceed the expectations of those utilizing or impacted by the department.
- Exhibits courtesy, compassion and respect to patients, families, visitors, physicians, administrators and co-workers.
- Knowledge about laws and regulations relating to job responsibilities, e.g., EEO, OSHA, patient rights, fraud/abuse laws.

- Work is performed in the Rehabilitation Department and on patient care units. Occasional exposure to blood, body fluids, infectious materials, anti-neoplastic pharmaceuticals and other materials and substances that may be used in patient care or on the patient care unit.
- Must be able to push, pull, and do heavy lifting without restrictions up to 51 pounds.
- Must have working knowledge of hoist equipment.
- Standing and walking for significant amounts of time is necessary.
- Communication skills required for directing staff, customer relations and marketing activities.
- Ability to perform documentation needs, manipulation of equipment, computer use and for direct patient care.
- Requires ability to perform as licensed clinician.
- Requires intense attention and concentration.
- Ability to cope with periodic stress associated with changing census and staffing demands.
- Demonstrated ability to prioritize and manage time effectively for self and others.
- Ability to deal with a wide range of personality types, ages and diverse cultural backgrounds.
Bachelor’s Degree in Physical Therapy, Occupational Therapy or Speech/Language Pathology is required, Master’s Degree is preferred.
- Current licensure in CT.
- Two to four years of clinical experience in a skilled nursing facility is required; one to two years of supervisory experience with multi-disciplines preferred.

Pay: $95,000.00 - $115,000.00 per year
