Established, fast paced single physician cardiology practice is seeking a compassionate and highly organized Registered Nurse with experience providing comprehensive nursing care in a fast-paced outpatient cardiology clinic. Skilled in managing high patient volumes while delivering exceptional patient-centered care, coordinating treatment plans, triaging urgent patient concerns, and collaborating with cardiologists and multidisciplinary healthcare teams. Proficient in cardiac assessments, patient education, medication management, prior authorizations, and electronic medical record documentation.
Responsibilities
- Perform comprehensive patient assessments, including vital signs, cardiac symptom evaluation, and review of medical histories.
- Triage high volumes of patient phone calls and electronic messages, identifying urgent cardiac concerns and coordinating appropriate interventions.
- Collaborate closely with cardiologists to develop, implement, and coordinate individualized treatment plans.
- Educate patients and families regarding cardiac conditions, medications, lifestyle modifications, diagnostic testing, and follow-up care.
- Review laboratory and diagnostic test results, communicate physician recommendations, and coordinate additional testing when indicated.
- Manage prescription refills, medication prior authorizations, and patient assistance programs while ensuring medication safety.
- Coordinate referrals to specialists, cardiac procedures, imaging, and hospital admissions as needed.
- Assist with in-office procedures including ECGs, blood pressure management, injections, and cardiac monitoring.
- Monitor anticoagulation therapy and provide patient education regarding medication compliance and safety.
- Maintain accurate and timely documentation in the electronic health record while ensuring compliance with HIPAA and regulatory standards.
- Coordinate care between hospitals, specialists, pharmacies, home health agencies, and primary care providers to ensure continuity of care.
- Respond efficiently to urgent patient needs while maintaining exceptional customer service in a high-volume clinical environment.
- Participate in quality improvement initiatives, patient safety programs, and workflow optimization.
- Maintain current knowledge of cardiology guidelines, medications, and evidence-based nursing practices.
