Prolific Care Homes 3 LLC is seeking compassionate, dependable, and motivated Caregivers to join our growing Adult Family Home team. Our 3–4 bed home provides high-quality, person-centered care to adults in a comfortable, home-like environment. We are looking for caregivers who are passionate about making a difference in the lives of others while maintaining professionalism and dignity.
Job Responsibilities
- Assist residents with activities of daily living (ADLs), including bathing, dressing, grooming, toileting, eating, and mobility.
- Administer medications according to Wisconsin DHS regulations and facility policies.
- Monitor residents for changes in condition and promptly report concerns.
- Complete accurate documentation and electronic charting.
- Prepare meals and assist with feeding when needed.
- Perform housekeeping, laundry, and maintain a clean, safe living environment.
- Encourage resident independence while providing compassionate support.
- Follow individualized care plans and resident rights.
- Attend required staff meetings and training.
- Follow all Wisconsin Adult Family Home (AFH) and DHS regulations.
Minimum Qualifications
- Must be at least 21 years of age.
- High School Diploma or GED preferred.
- Previous caregiving, CBRF, CNA, DSP, or healthcare experience preferred but not required.
- Reliable transportation and dependable attendance.
- Strong communication and teamwork skills.
- Ability to lift, transfer, bend, and assist residents safely.
- Must pass a Wisconsin Caregiver Background Check.
Required Certifications
Applicants should have, or be willing to obtain upon hire:
- Current TB Test (2-Step TB Skin Test or approved TB Blood Test)
- CBRF Standard Precautions
- CBRF Fire Safety
- CBRF First Aid & Choking
- CBRF Medication Administration
- Any additional training required by Wisconsin DHS for Adult Family Home caregivers
