Greater Meridian Health Clinic, Inc. is seeking dedicated and highly skilled Licensed Practical Nurse (LPN) to provide compassionate, comprehensive nursing care across diverse healthcare settings. The ideal candidate will possess a strong foundation in patient assessment, medication administration, and medical documentation, with experience working with various patient populations including geriatric, pediatric, and individuals with disabilities. As an LPN, you will play a vital role in supporting patient health, ensuring safety, and delivering quality care in accordance with regulatory standards and best practices.
Responsibilities
- Perform thorough patient assessments, including vital signs, physical examinations, and health histories to inform care plans.
- Administer medications and injections accurately while adhering to prescribed protocols and safety guidelines.
- Maintain detailed medical records using electronic health record (EHR) systems such as EClinicalWorks to ensure precise documentation of patient care.
- Provide specialized care for patients with conditions such as Alzheimer's disease, dementia, or those requiring memory care or hospice services.
- Collaborate with interdisciplinary teams to develop discharge plans and coordinate long-term or post-acute care for patients in skilled nursing facilities or home & community settings.
- Ensure compliance with HIPAA regulations and maintain strict confidentiality of patient information at all times.
Skills
- Strong knowledge of medical terminology, physiology, ICD coding (ICD-9/10), and diagnostic evaluation techniques.
- Proficiency in EMR/EHR systems such as eClinicalWorks for accurate medical documentation and data management.
- Ability to perform physical examinations and assessments with attention to detail; experience with specialized care such as geriatrics or pediatric populations is advantageous.
- Familiarity with long-term care regulations, infection control policies, and clinical confidentiality policies.
- Excellent communication skills for effective triage and patient education; experience working with individuals with developmental disabilities or behavioral health needs is a plus.
- Knowledge of advanced nursing practices including acute care procedures like spinal taps or gastric lavage; familiarity with medical imaging or surgical assistance is beneficial.
