Position Overview:
The Designated Coordinator (DC) under Minnesota Statutes, section 245D.081, is responsible for ensuring the delivery of person-centered services in compliance with Minnesota Statute 245D and Home and Community-Based Services (HCBS) standards. The DC oversees service implementation, ensures regulatory compliance, trains and supervises staff, and advocates for clients. This role includes facilitating client intakes, supporting employee onboarding, and serving as a temporary backfill for client services when necessary. The DC plays a vital role in promoting the mission and values of Infinity Care Solutions LLC.
Key Responsibilities:
Service Coordination and Oversight
· Develop, implement, and monitor Individual Service Plans (ISPs) to ensure alignment with clients' assessed needs, goals, and preferences.
· Review and accept Coordinated Service and Support Plans (CSSPs) and Service Agreements (SAs) for all clients.
· Conduct semi-annual and annual reviews of client progress, including updates to plans and required assessments.
· Facilitate interdisciplinary team meetings to ensure effective communication among clients, families, staff, and external providers.
· Coordinate transition and discharge planning, ensuring proper documentation and support.
· Serve as the official representative of the license holder with DHS, acting as the primary point of contact to receive and respond to all communications, legal notices, and compliance orders, while ensuring accurate and timely submission of licensing applications, updates, and disclosures to maintain full regulatory compliance.
Staff Supervision and Training
· Lead employee onboarding, including orientation and training on 245D standards, HCBS procedures, and person-centered practices.
· Supervise and mentor Direct Support Professionals (DSPs) to ensure quality service delivery.
· Provide ongoing training on regulatory updates, safety protocols, and best practices.
· Evaluate staff performance through regular assessments and feedback to ensure compliance and service excellence.
Regulatory Compliance and Quality Assurance
· Maintain up-to-date client and organizational records in compliance with 245D standards and HCBS policies.
· Complete and submit required documentation, including incident reports and progress notes, within mandated timeframes.
· Participate in audits, inspections, and quality improvement initiatives.
· Ensure adherence to state and federal regulations, as well as organizational policies.
Client Advocacy and Support
· Advocate for clients’ rights, preferences, and goals in all service planning and delivery.
· Address client and family concerns promptly, ensuring timely resolution.
· Promote a safe, respectful, and inclusive environment for clients and staff.
Communication and Customer Service
· Act as a liaison with case managers, families, and other stakeholders to address inquiries, concerns, and service needs.
· Monitor and respond promptly to client questions, complaints, and service requests.
· Build and maintain positive relationships with case managers and community partners.
Documentation and Administrative Duties
· Ensure timely completion of employee timesheets and obtain client signatures as required.
· Keep employee files current with hiring and training documentation.
· Update client records with required documentation, maintaining strict confidentiality.
· Track inquiries, referrals, and client transitions, ensuring proper documentation and follow-up.
Operational Support
· Serve as a temporary backfill for clients while hiring and onboarding new staff.
· Assist with outreach, marketing, and recruitment efforts as needed.
· Keep the Executive Director informed of operational needs and concerns.
Education and Experience Requirements:
· Bachelor’s degree in social work, psychology, special education, case management, or a related human services field (required).
· Equivalent work experience may be considered in lieu of a degree if permitted by applicable regulations.
Experience:
· Minimum of two years of experience working with individuals with disabilities or providing services regulated under 245D standards.
· Experience as a case manager, social worker, or designated coordinator is strongly preferred. Supervisory or leadership experience is also preferred.

Certifications/Licenses:
· Valid driver’s license and reliable transportation.
Skills and Qualifications:
· Thorough knowledge of 245D HCBS regulations and person-centered service requirements.
· Strong organizational and time-management skills.
· Excellent written and verbal communication skills.
· Ability to lead, train, and motivate staff effectively.
· Proficiency in using electronic health records (EHRs) and other relevant software systems.
· Driving is required as part of this job. Applicants must provide a valid driver’s license and a driving record for the past three years.
· Experience in Case management including creating support plan.
Physical Requirements:
· Ability to perform light physical tasks, including assisting clients with mobility or daily living activities as needed.
· Ability to travel between client locations and office sites as required.
