Through the Washington State Health Care Authority’s Hub and Spoke Model of Care, you’ll play a key role in connecting people to life-saving treatment and recovery supports.
As a Spoke Care Navigator, you will be the vital link between clients, healthcare providers, and community resources. Working closely with the Providence “Hub” and the NEWABH “Spoke,” you’ll coordinate care, support MAT patients in recovery, and ensure smooth transitions across behavioral health, medical, and social service systems.
