Ragaad Home Health Care has been providing person-centered services since July 2020. What started as a small program has continued to grow over the past six years, and today we support individuals and families in communities throughout Minnesota.

Role:
Ragaad Home Health Care is seeking a motivated and community-focused OutreachCoordinator to help expand our 245D Individualized Home Supports (IHS) services.
This is a 10 hour per/week position focused primarily on community outreach, developing referral relationships, identifying individuals who may benefit from our services, and helping connect new clients with Ragaad Home Health Care.
The Outreach Coordinator will represent our organization professionally in the community and build relationships with case managers, social workers, counties, families, guardians, and other community partners.
Responsibilities:
- Conduct outreach to identify individuals who may qualify for and benefit from 245D IHS services.
- Develop and maintain relationships with county case managers, social workers, care coordinators, guardians, families, and community organizations.
- Introduce Ragaad Home Health Care and explain the 245D/IHS services we provide.
- Follow up with potential referrals and referral sources.
- Help coordinate the initial referral and intake process for new individuals.
- Work with management to understand each person's needs, preferences, authorized services, and level of support.
- Assist the organization in finding appropriate staff who can be matched with the individual based on their needs and preferences.
- Attend community meetings, networking opportunities, and outreach events when appropriate.
- Maintain professional communication and confidentiality.
- Keep accurate records of outreach activities, referrals, contacts, and follow-up.
- Represent Ragaad Home Health Care's commitment to person-centered, respectful, and dignified services.
Qualifications
- Experience in outreach, healthcare, social services, disability services, case management, marketing, or a related field is preferred.
- Knowledge of Minnesota 245D services and Individualized Home Supports (IHS) is strongly preferred.
- Familiarity with county referral systems, case managers, and waiver services is a plus.
- Strong communication and relationship-building skills.
- Comfortable reaching out to new referral sources and developing professional relationships.
- Organized, dependable, and able to work independently.
- Must understand the importance of confidentiality, professional boundaries, dignity, and person-centered services.
- Experience working with individuals with disabilities or vulnerable adults is preferred.
