Long Term Care Nursing Supervisor/MDS Coordinator

Principal Purpose of the Job:
The purpose of this position is to ensure that optimal quality care is delivered to each resident of WCHC LTC Unit. Maintain the disciplines to guarantee that quality of life procedures are integrated with quality of care procedures. Accept responsibility and accountability for the functions and activities of the nursing staff. Coordinate and oversee all functions of the MDS and resident care assessment process to assure compliance with Federal and State regulations, and to certify timely completion of MDS assessments.
Essential Functions of the Job:
- Develop, Coordinate, and evaluating direct and indirect care to residents.
- Establish, manage and monitor written resident care plans including preliminary and comprehensive objectives
- Assures compliance with established departmental policies and procedures, continuous quality improvement, safety, environmental and infection control standards.
- Cooperates with Medical staff, nursing personnel and ancillary staff to maintain excellent resident care delivery, encourage good employee relations and meet all departmental objectives.
- Monitors, advocates, and coordinates the wellbeing of all resident health issues, care issues, and concerns.
- Completes the MDS, CAA and Care plan requirement of our residents under the guideline of Centers for Medicare and Medicaid Services.
- Maintains continuing education of the MDS system
- Monitors delivery of care by coordinating Dr’s rounds, Medical staff reports, Care plan meetings, fall meetings.
- Monitors delivery of care by identifying desired resident care outcomes, intervene if outcomes deviate from expected plan of care.
- Attends other required meetings per approval of DON/ CEO.
- Follows all regulatory requirements for CMS, KDADS, and CAH regulations.
- Follows all nurse practice acts.
- Review and coordinates diagnostic and therapeutic procedures as needed.
- Recognizes and reports safety hazards.
- Develops an interdisciplinary care plan for resident care, implement those resident care plans, and monitors for changes and updates care plan appropriately
- Communicates resident care needs to other personnel involved in the care of residents and patients.
- Recognizes impending emergencies and acts appropriately and calmly.
- Recognizes resident changes in condition and reports concerns to Medical staff and families.
- Communicates clearly and accurately with residents, families, medical staff, licensed nurses, CNA’s and ancillary staff.
- Monitors appropriate EMR entries and all other required documentations.
- Assists licensed nurses, or medical staff as requested.
- Admits, transfers, and dismisses residents.
- Cultivate a culture of resident centered care: not task oriented care
- Expected to documentation on each resident updated and current.
- Expected to train on call system and add pendants, monitor reports, and troubleshoot system
- Demonstrates sensitivity to patients or residents care, comfort and privacy.
- Reports any concerns of emotional instability, behavioral issues, combative or inappropriate behavior to medical personnel.
- Provide caring environment and expect all staff to care for residents in a kind, compassionate manner. Being careful to preserve patient and resident’s dignity.
- Comply with scheduling guidelines, and assist if there is a staffing shortfall.
- Assist with chart review, maintain a Quality control process.
Additional Responsibilities and Objectives:
- Follow and enforce Residents’ Rights policies at all times.
- Follow Abuse Neglect and Exploitation Policy and monitor for any concerns at all times.
- Assume accountability for data contained in the employees’ handbook.
- Participate in training and education as requested and complete Care Learning courses annually
- Come to work as scheduled and consistently demonstrate dependability and punctuality.
- Cooperate and work together with all co-workers.
- Promote positive public relations with patients, family members, and guests of WCHC.
- Uses tactful and appropriate communication in all situations.
- Understand and comply with the confidentiality of all WCHC information.
- Report to work in neat, clean attire and consistently present an appropriate professional appearance.
- Assumes responsibility for own professional growth and development, plans and participates in conferences, classes and in-services to improve knowledge and practice in resident care.
- Meets safety needs by observing infection control practices, Quality control, oxygen safety and environmental safety reporting concerns to appropriate personnel.
- Understand the risk of bloodborne diseases, use and enforce use of appropriate personal protective equipment by staff.
- Handle hazardous chemicals in a safe, efficient and effective manner in compliance with both Federal and State ordinances.
- Share knowledge with others. Act as a role model and mentor to new staff. Assist with orientation of new personnel.
Physical Requirements:
This position demands good physical and mental health. The LTC Supervisor daily routine will consist of computer work, but may also include lifting and positioning of residents weighing at a minimum of 150 pounds. Must be able to see and hear with or without corrective measures that will enable these senses to function adequately to ensure that the requirements of the position can be fully met. Work may also require pushing or transporting residents in a wheel chair. The LTC Supervisor must be able to move about sufficiently to respond to life threatening emergencies including performing CPR and assisting with evacuation of residents during emergency situations.
Qualifications, Education and Experience:
Current RN license,
Maintain proficiency in the MDS 3.0 certification.
CPR Required
Ability to assess and respond to the needs of residents and families in a home setting.
Skilled in nursing practice.
Ability to cope with staff, resident, family emotional stress.
Tolerant of individual lifestyles.
Must be able to maintain confidentiality regarding patient and company proprietary information.
Must have the ability to relate professionally and positively and work cooperatively with residents, families, and other employees at all levels.
Must possess the necessary computer skills at a level necessary to accomplish the job.
Must be capable of maintaining a regular attendance.
