Responsibilities:
- Conduct client assessments to determine appropriate care and treatment plans
-Coordinate appointments with providers
- Collaborate with healthcare professionals to coordinate and manage client needs
- Monitor and evaluate client progress, adjusting care plans as needed
- Utilize case management software to document patient information and track progress
- Answer incoming calls
Skills:
- Strong client assessment skills
- Familiarity with case management principles and practices
- Ability to multi-task
- BILINGUAL a PLUS
