We are seeking a highly organized, disciplined, and self‑motivated HCBS Home Care Case Manager to support clients receiving Home and Community‑Based Services (HCBS). This position offers an exciting opportunity to make a meaningful difference in the lives of those needing specialized care throughout Colorado Springs, El Paso Counties, Denver Metro area. This role requires exceptional communication skills, strong professional judgment, and the ability to collaborate across diverse educational levels, learning styles, and family dynamics.

Key Responsibilities
- Coordinate and manage client care plans in alignment with HCBS program requirements, agency policies, and regulatory standards.
- Facilitate communication between clients, families, guardians, hospitals, providers, and insurance entities to ensure seamless care transitions and service continuity.
- Maintain strict HIPAA compliance, protecting all client information and ensuring confidentiality at every stage of service delivery.
- Conduct regular client check‑ins, home visits, and care plan reviews to ensure needs are met and services remain appropriate.
- Provide coaching, guidance, and training to caregivers and support staff to reinforce quality, compliance, and best practices.
- Document all case activity accurately and in a timely manner within agency systems and state‑required platforms.
- Support crisis intervention, problem‑solving, and advocacy for clients and families.
- Promote client independence, safety, and dignity through person‑centered planning and compassionate engagement.
- Conduct initial assessments and annual client reviews to evaluate needs, update care plans, confirm service appropriateness, and support continued eligibility and compliance.
Qualifications
- Experience in HCBS, Home Care, PASA, or related human services fields required.
- Demonstrated ability to communicate effectively with clients, families, guardians, and multidisciplinary teams.
- Strong background in communication with clients and guardians.
- Ensure compliance with HIPAA regulations by maintaining confidentiality and protecting patient information at all times.
- Excellent communication skills for effective collaboration with caregivers, clients, and guardians.
- Strong organizational skills with the ability to manage multiple cases, deadlines, and priorities.
- Ability to work independently with minimal supervision while maintaining accountability and follow‑through.
- Experience coaching, training, or leading staff is highly valued.
- Familiarity with Colorado HCBS programs, Medicaid processes, and documentation standards is a plus.
- Valid driver’s license and reliable transportation for client visits.
- Must pass background checks required by state regulations.
- Proficient in Google Workspace; Microsoft Copilot
