Overview
Correct Rx Pharmacy Services, Inc., a recognized leader in institutional clinical pharmacy services, is seeking a motivated clinical pharmacist to deliver innovative clinical programs that improve outcomes in a direct patient care setting. This position provides an excellent opportunity to work collaboratively on a multi-disciplinary healthcare team.
Responsibilities and Duties
- Business and client relationship building
- Evaluate, monitor and enhance medication therapy and care planning
- Provide Pharmacy & Therapeutics (P&T) Meetings
- Complex drug utilization review and analysis
- Disease state management
- Drug Information
- Formulary management
- High acuity patient case reviews
- Participate in on-going administrative meetings
- Patient education
- Provider consultations and education
- Regulatory adherence
- Healthcare technology integration
- Travel for client/site visit
Qualifications
- Licenses: Pharmacist License in Michigan
- Education level: Pharm.D.
- Experience: At least one year of post graduate residency training or psychiatric practice preferred
- Certifications: CPR, Board certification through the Board of Pharmacy Specialties is preferred
