Your Role:
- Conduct physical exams, manage both acute and chronic conditions.
- Manage medication regimens and polypharmacy.
- Develop comprehensive care plans including collaboration with primary care physicians, home health or hospice services.
- Offer personalized patient and family education and support, with on-call availability for nursing facility staff.
Who We’re Looking For:
- APRN licensed in Texas, with effective communication skills and experience in geriatric, adult, acute, or family medicine.
- Professionals eager to contribute to a team while capable of independent decision-making.
- Candidates passionate about improving elderly patient care, demonstrating flexibility and initiative.
