Responsibilities
- Conduct comprehensive patient assessments including vital signs, physical examinations, and health history reviews to determine immediate needs and ongoing care plans.
- Administer medications, injections, IV infusions, and wound care with precision following agency protocols.
- Utilize electronic health record (EHR) systems like Alora to document medical records accurately, ensuring compliance with HIPAA regulations.
- Collaborate with interdisciplinary Physicians, PT, OT, RT, Social work, NA teams in home health care environments .
- Provide patient education on medication administration, pain management strategies, nutrition plans, and admission discharge instructions to promote recovery independence, and well-being.
- Requirements Valid nursing license with current registration; additional certifications such as BLS (Basic Life Support), ACLS (Advanced Cardiac Life Support), or PALS (Pediatric Advanced Life Support) preferred.
- Demonstrated experience working with diverse populations including pediatrics, geriatrics, individuals with developmental disabilities or memory care needs.
- Proficiency in EMR/EHR systems such as Alora; familiarity with ICD coding (ICD-9/ICD-10) is advantageous.
- Excellent communication and patient‑education abilities
- Ability to work independently in a home‑based setting
- Home health experience preferred but not required experience, wound care is a plus.
- Knowledge of infection control practices and to maintain safety standards across all patient interactions.
- Strong communication skills for effective collaboration within multidisciplinary teams and clear patient education.
- Ability to demonstrate empathy compassion excellent clinical judgment and attention to detail.
