The Licensed Practical Nurse (LPN) provides direct resident care, monitors health and vitals, and works closely with Registered Nurses (RNs) and Unit Managers to ensure high-quality, individualized care.
Deliver direct personal care to residents
Observe, assess, and document resident health and vital signs
Assist RNs and Unit Managers with care planning and medical histories
Maintain accurate documentation in compliance with company and state regulations
Promote a professional, collaborative, and resident-focused environment
Current Licensed Practical Nurse (LPN) license in Massachusetts
Long-term care experience preferred but not required
Strong communication, teamwork, and organizational skills
Dedication to providing compassionate, high-quality care
