Job Overview

Duties
- Assist patients with activities of daily living (ADLs), including bathing, dressing, grooming, and mobility support
- Monitor patient vital signs such as blood pressure, pulse, and temperature to track health status accurately
- Support medication administration following care plans and HIPAA regulations to ensure safety and compliance
- Provide personal transportation assistance for appointments, errands, or community outings
- Prepare nutritious meals tailored to individual dietary needs and assist with light housekeeping tasks such as cleaning and laundry
- Utilize patient transfer methods like Hoyer lifts or manual handling techniques to ensure safe mobility support
- Observe and document changes in patient condition, including memory care needs related to Alzheimer’s or dementia, ensuring timely communication with healthcare professionals
Experience
- Prior experience working with individuals with disabilities, developmental or intellectual disabilities support, or senior care is highly preferred
- Demonstrated experience in home health, assisted living, nursing homes, or group home settings
- Knowledge of patient observation techniques, medication management, and care plan implementation
- Familiarity with specialized care such as Alzheimer’s care, dementia care, hospice care, or neurodevelopmental disorders
- Certification in CPR and First Aid is preferred but not required; additional certifications like HIPAA training or manual handling are advantageous
- Strong communication skills for effective client interaction and collaboration with healthcare teams
