Camellia Gardens Center for Nursing & Rehabilitation is seeking a dedicated Wound Care Nurse (RN/LPN) to join our skilled nursing team and play a vital role in promoting skin health, preventing complications, and enhancing the quality of life for our residents.

The Wound Care Nurse is responsible for assessing, treating, and monitoring resident skin integrity and wound care needs. This role works closely with nursing staff, physicians, residents, families, and the interdisciplinary team to develop effective treatment plans and ensure the highest standards of care.
Perform comprehensive skin assessments, preventive skin care, wound treatments, and dressing changes according to established protocols.
Monitor and promote proper turning and repositioning practices to prevent skin breakdown.
Assess, document, and communicate changes related to pressure injuries, wounds, and skin conditions to physicians, residents, and families as appropriate.
Collaborate with physicians and interdisciplinary team members regarding wound care plans and treatment recommendations.
Participate in physician rounds as needed to discuss residents receiving wound care or specialized treatments.
Monitor residents with pressure injuries and evaluate nutritional and hydration needs to support healing and recovery.
Collaborate with the dietary team and initiate appropriate interventions when additional nutritional support is needed.
Maintain accurate and timely wound care documentation in accordance with regulatory requirements and facility policies.
Maintain a clean, organized treatment cart and ensure adequate availability of wound care supplies.
Follow infection prevention and control policies and maintain a safe care environment.
Participate in quality improvement initiatives and other nursing projects as assigned.
Current Registered Nurse (RN) or Licensed Practical Nurse (LPN/LVN) license in the state of practice.
One to three years of nursing experience preferred; skilled nursing or wound care experience strongly preferred.
Current CPR certification required.
Strong clinical assessment, critical thinking, and documentation skills.
Knowledge of wound prevention, treatment protocols, and skin integrity management.
Excellent communication and interpersonal skills.
Ability to work independently while collaborating effectively with residents, families, and the healthcare team.
