As a Case Manager with Camelback Recovery/Phoenix Residential, you will deliver compassionate and professional service, while providing unconditional support to the individuals we serve. In this role, you will be responsible for maintaining a caseload of clients by meeting with clients weekly and tracking the clients progress. In addition, you will work closely with the Clinical Director to manage internal databases, maintain client files and profiles, and assist in other daily business operations.
Coordinate and provide care that is safe, timely, effective, efficient, equitable, and client-centered
Handle case assignments, draft treatment plans, review case progress and determine case closure
Help clients achieve wellness and autonomy
Help patients make informed decisions by acting as their advocate regarding their clinical status and treatment options
Develop effective working relations and cooperate with medical team throughout the entire case management process
Arrange abeyances with social services, health and government agencies
Take the extra mile and interact with patients to keep track of their progress and to ensure satisfaction
Record cases information, complete accurately all necessary forms and produce reports for referral sources
Promote quality cost-effective interventions and outcomes
Assess and address motivational and psychosocial issues
Adhere to professional standards as outlined by protocols, rules and regulations
Maintain data regarding client progress, including EMR upkeep, report writing, and shift note tracking
Coordinate with senior management and report on ongoing cases and new assignments
Attend staff meetings and training as required

One year experience in mental health, substance abuse, and case management

Must have valid Arizona Level 1 Fingerprint Clearance Card
Must have valid Driver's License
Must have high school diploma or GED
Bachelor’s degree in social services, sociology, or psychology strongly preferred
Must be 21 years of age or older
Must have Knowledge of Windows | Basic Email | Microsoft Word | Timekeeping software | Google & basic computer navigation skills.
Must hold an active CPR/First-Aid/AED certification
If applicable, must have one year of maintained sobriety
Must be able to pass drug test
