Social Worker

A skilled nursing and long-term care facility in the St. Paul area is seeking a Social Worker to support the psychosocial needs of residents and their families across admission, care, and discharge. Prior long-term care experience is what matters most to us for this role.
What you'll do:
Conduct psychosocial assessments on admission and at required intervals
Develop and update care plans addressing residents' social and emotional needs
Coordinate discharge planning and connect residents and families with community resources
Advocate for resident rights and help resolve concerns and grievances
Support residents and families through adjustment, coping, and end-of-life care
Collaborate with the interdisciplinary team and contribute to MDS assessments
Maintain documentation and ensure compliance with state and federal social services regulations
What we're looking for:
Prior social services experience in a skilled nursing or long-term care setting. This is our top priority and the main factor in the offered rate.
Bachelor's or Master's degree in Social Work
Current Minnesota social work licensure (LSW, LGSW, LISW, or LICSW)
Working knowledge of long-term care regulations, MDS, and discharge planning
Strong interpersonal, communication, and organizational skills
Compensation and benefits:
$28 to $35 per hour, based on experience
