Job Overview
A Medicare-certified home health agency is seeking an organized and dependable Licensed Practical Nurse (LPN) to join its office team as an LPN Case Manager. This is an office-based position focused on coordinating home health services, managing patient schedules, monitoring clinical documentation, and communicating with patients, clinicians, physicians, and other members of the care team.
The ideal candidate is detail-oriented, comfortable working in a fast-paced environment, and able to manage multiple patients and priorities while ensuring services are scheduled and completed appropriately. Previous home health or case management experience is preferred but not required for the right candidate.
Key Responsibilities
- Coordinate patient care and serve as a point of communication between patients, families, field clinicians, physicians, and office staff.
- Schedule and manage skilled nursing and therapy visits according to each patient’s plan of care and required visit frequency.
- Monitor patient schedules to identify missed, omitted, overdue, or incorrectly scheduled visits.
- Communicate scheduling changes and patient needs promptly to the appropriate clinicians and office staff.
- Review clinical documentation for completeness, accuracy, and timely submission.
- Follow up with clinicians regarding missing documentation, incomplete notes, scheduling concerns, and other outstanding items.
- Maintain accurate patient and clinician information within the electronic medical record and referral systems.
- Participate in case conferences and assist with monitoring patient progress, changes in condition, recertifications, resumptions of care, transfers, and discharges.
- Communicate changes in a patient’s condition or plan of care to the appropriate registered nurse, physician, or other healthcare provider.
- Assist with obtaining and tracking physician orders and other required clinical documentation.
- Help ensure services are provided in accordance with physician orders, the established plan of care, agency policies, and applicable home health requirements.
- Respond professionally to telephone calls and messages from patients, caregivers, clinicians, physicians, and referral sources.
- Assist with referral coordination and the transition of new patients into home health services.
- Maintain patient confidentiality and comply with HIPAA requirements.
- Complete additional clinical and administrative duties related to patient care coordination as assigned.
Qualifications
- Current and unrestricted Florida LPN license.
- Current Basic Life Support (BLS) or CPR certification.
- Previous home health, case management, medical office, or patient-care coordination experience preferred.
- Familiarity with home health regulations and documentation requirements is preferred.
- Experience with Axxess Home Health or another electronic medical record system is a plus.
- Strong computer skills, including proficiency with email and Microsoft Office.
- Excellent written and verbal communication skills.
- Strong organizational, time-management, and problem-solving abilities.
- Ability to manage multiple patients, schedules, deadlines, and competing priorities.
- Ability to communicate professionally and effectively with patients, families, clinicians, physicians, and coworkers.
- Dependable attendance and the ability to work effectively as part of an office-based team.
- High level of accuracy and attention to detail.

Pay: $50,000.00 - $54,000.00 per year
