Responsibilities
- Provide skilled nursing care according to the physician's orders and plan of care
- Administer medications and treatments
- Monitor patient conditions and report changes to the supervising RN
- Maintain accurate and timely documentation
- Educate patients and caregivers as appropriate
- Promote patient safety, comfort, dignity, and independence
- Collaborate with the interdisciplinary care team
Qualifications
Required
- Active Indiana Licensed Practical Nurse (LPN) license
- Current CPR certification
- Reliable transportation
- Strong communication and documentation skills
- Ability to work independently in a home setting
Preferred
Experience with one or more of the following:
- Home Health or Private Duty Nursing
- Tracheostomy care
- G-Tube care
- Catheter care
- Respiratory treatments
- Neurological conditions
- Spinal cord injury care
- Quadriplegia care
- Long-term care or skilled nursing
Training may be available for qualified candidates with strong nursing experience.
