Duties
- Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts when necessary
- Monitor patients’ health by taking vital signs, observing changes in condition, and reporting findings accurately to healthcare professionals
- Prepare nutritious meals tailored to individual dietary needs and assist with meal planning and cooking to promote health and well-being
- Administer medications following care plans while adhering to HIPAA regulations to ensure patient confidentiality and safety
- Maintain a clean and safe environment through regular cleaning tasks, laundry assistance, and organization of personal spaces
- Support residents in assisted living or long-term care facilities by fostering social engagement and emotional support
- Document patient observations, update care plans regularly, and ensure compliance with all healthcare standards including CPR and first aid certifications
Experience
- Prior experience working in group homes, nursing homes, assisted living facilities, or home & community care environments preferred
- Demonstrated knowledge of Alzheimer’s care, dementia care, memory care, hospice care, or working with individuals with developmental disabilities or autism
- Hands-on experience with patient monitoring techniques such as vital signs measurement and medication administration
- Familiarity with using assistive devices like Hoyer lifts or resident’s <PERSON>; for safe transfers and mobility support
- Strong background in caregiving roles involving heavy lifting, patient observation, and working within care plans under HIPAA guidelines
- Experience in home health services or social work that emphasizes compassionate patient support and community engagement
- Certifications such as CPR, first aid, or nursing assistant credentials are highly valued to ensure safe and effective patient care
