Licensed Master Social Worker (LMSW) — Hospice & Home Health (Texas)
Job Summary:
We are seeking a compassionate Licensed Master Social Worker (LMSW) to join our company Skye Hospice and Skye Home Health. The LMSW provides psychosocial support to patients and families, assists with care planning, and connects clients with community resources to support quality of life.
Responsibilities:
- Complete psychosocial assessments for hospice and home health patients
- Provide emotional support, counseling, and crisis intervention
- Assist with advance care planning and end-of-life support
- Coordinate services with the interdisciplinary care team
- Connect patients and families to community resources
- Maintain timely and accurate documentation
- Travel as needed to Eagle Pass, Uvalde, Del Rio
Requirements:
- Master’s degree in Social Work (MSW)
- Active Texas LMSW license
- Experience in hospice, home health, medical social work, or behavioral health preferred
- Strong communication, empathy, and organizational skills
- Reliable transportation for patient visits
