KARE CENTER in Ann Arbor, MI is seeking a dedicated and skilled General Practitioner (GP) to join our growing multidisciplinary healthcare team. In this role, you will provide comprehensive primary care services including patient evaluations, diagnosis, treatment planning, chronic disease management, and preventive care. You will work alongside psychiatrists, nurse practitioners, physician assistants, therapists, and social workers to deliver integrated, whole-person care in a collaborative and supportive environment.
- Conduct comprehensive patient evaluations including history, physical examinations, and diagnostic assessments.
- Diagnose and manage a wide range of acute and chronic medical conditions across all age groups.
- Develop and implement individualized treatment plans incorporating pharmacotherapy, lifestyle modifications, and referrals.
- Prescribe and manage medications, monitoring patient response and adjusting treatment as clinically indicated.
- Provide preventive care including health screenings, immunizations, and wellness counseling.
- Coordinate care with specialists, behavioral health providers, and community resources.
- Conduct follow-up appointments to monitor patient progress and overall health and well-being.
- Assess and manage urgent and emergent clinical situations, coordinating appropriate levels of care as needed.
- Maintain accurate, thorough, and timely clinical documentation in the electronic health record (EHR).
- Participate in interdisciplinary team meetings, quality improvement initiatives, and continuing education.
- MD or DO degree from an accredited medical school.
- Completion of a Family Medicine, Internal Medicine, or General Practice residency program.
- Board Certified or Board Eligible in Family Medicine or Internal Medicine.
- Active Michigan medical license (or eligibility to obtain) and DEA registration.
- Experience in primary care, urgent care, or integrated healthcare settings preferred.
- Strong diagnostic, clinical, and communication skills.
