Position Summary:
Blue Point Nursing and Rehab Center is seeking a compassionate and dedicated Social Worker Designee to join our interdisciplinary team. In this role, you will support residents and their families by addressing psychosocial needs and ensuring the highest quality of care and quality of life.
Responsibilities:
Conduct psychosocial assessments and develop care plans
Provide counseling and emotional support to residents and families
Coordinate discharge planning and transitions of care
Act as a liaison between residents, families, and care teams
Assist with admissions and resident adjustment
Ensure compliance with state and federal regulations
Qualifications:
Licensed Social Worker (LSW/LCSW) in Maryland
Experience in a skilled nursing facility preferred
Strong communication and interpersonal skills
