We are seeking Dependable, Experienced Caregivers with previous experience to assist our clients in their home.
We are also accepting family & friend caregivers who are new to the Waiver Program, as well as clients and caregivers who are already enrolled and looking for a new agency.
Pay Rate ranges between $11-$13 per hour.

A successful candidate will have the following:
- 18 years of age or older
- High School Diploma or GED
- Prior experience working for a Home Care Agency, Nursing Home or in a Healthcare Setting.
- 2 Step TB or Quantiferon Gold Blood Test within the past 12 months or willing to obtain.
- Willing to obtain state regulation background check/clearances
Responsibilities:
- Provide routine individualized care to the elderly or persons with disabilities
- Monitor and report changes in health status
- Provide personal care, such as bathing and dressing
- Light Housekeeping
- Prepare and Serve meals
- Medication Reminders

What is the Community Health Choices Waiver program?
The Community Health Choices Wavier Program, often called home and community-based services (HCBS waivers), is a Medicaid program for eligible individuals who require long-term care services but prefer to stay in their homes instead of institutional settings like nursing homes. The program promotes independence, choice, and quality of life by allowing individuals to receive ongoing care while staying in the comfort of their homes.
Is my loved one eligible?
If your friend, family member or loved one has Medicaid insurance and is 18 or over, they may be eligible for the Community Health Choices Waiver Program. A Power of attorney, spouse or payee is not eligible to be a paid caregiver
