OT Occupational Therapist - PRN or Independent Contractor
Occupational Therapist needed to make home visits in Longmont / Boulder / Erie or surrounding areas for home health care clients. Position is PRN.
Requirements:
Occupational Therapist with experience in Home Health, rehab, SNF, outpatient, correctional facility, schools, medical center, Float and pool therapists welcome!
Responsibilities and Duties include:
- Make home visits and provides the care ordered by the physician.
- Complete required documentation that makes evident patient/family needs, interventions, and progress in accordance with clinical, quality, and regulatory requirements. Assures that all documentation is available in the electronic record within the deadlines.
- Assure that patient and family needs are identified, prioritized, and met in order to achieve desired clinical outcomes.
- Collaborate with appropriate team members to develop and implement patient-specific plans of care.
Certifications:
- Driver’s License
- Must have a current CO OT license.
