The Team Lead provides oversight and supervision to staff and is responsible for coordinating comprehensive services for individuals supported by the program. This position combines leadership, administrative oversight, and direct service responsibilities to support high-quality, person-centered care and ensure compliance with organizational and regulatory requirements.
Provide first-line supervision to direct support staff through coaching, mentoring, training, and performance management.
Organize, schedule, and distribute work assignments among assigned personnel.
Monitor staff performance and provide ongoing feedback to support professional development.
Ensure staff understand and comply with agency policies, procedures, and program expectations.
Communicate pertinent information regarding program operations, policy updates, and organizational initiatives.
Participate in hiring, onboarding, orientation, and staff training activities as assigned.
Address employee concerns and collaborate with program leadership to resolve personnel issues.
Maintain an assigned direct service caseload.
Coordinate and oversee medical appointments and healthcare needs of individuals supported.
Attend medical appointments when necessary and communicate with healthcare providers regarding individuals' needs and treatment plans.
Maintain positive working relationships with individuals supported, family members, guardians, healthcare providers, staff, and external stakeholders.
Support individuals in achieving goals identified in their person-centered plans.
Advocate for individual choice, self-determination, dignity, and community inclusion.
Actively participate in the person-centered planning process by:
Attending planning meetings.
Providing input and recommendations.
Coordinating the development of training plans.
Ensuring all required documentation is completed accurately and timely.
Reviewing daily notes and progress documentation.
Monitoring implementation of person-centered plans.
Completing monthly reviews and follow-up activities.
Communicating with service coordinators and other team members.
Prepare, review, and submit reports and documentation related to individuals supported by the program.
Ensure compliance with all applicable regulations, licensing standards, funding requirements, and agency policies.
Participate in quality assurance and continuous quality improvement activities.
Assist with audits, inspections, and corrective action plans as needed.
Maintain accurate and confidential records.
Participate in meetings, trainings, and committees as assigned.
Perform other duties as assigned.
Required:
Current Certified Psychiatric Rehabilitation Practitioner (CPRP) Certification.
Valid Pennsylvania Driver's License with a minimum of three (3) years of driving experience acceptable to RHD's insurance requirements.
Bachelor's Degree in Human Services or a related field, including but not limited to sociology, psychology, counseling, rehabilitation, nursing, special education, or social work;

OR
High School Diploma or GED and five (5) years of relevant experience supporting individuals with intellectual disabilities, behavioral health challenges, or related populations.
Ability to obtain and maintain required clearances, including Pennsylvania Child Abuse Clearance, Pennsylvania State Police Criminal Record Check, and FBI Criminal History Clearance.
Ability to successfully complete all required trainings.
Ability to safely operate agency vehicles when required.
