Job Summary
We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our Personal Care Unit. The ideal candidate will possess a strong foundation in patient care, with experience in diverse healthcare settings including long-term care, assisted living, and acute care environments. As an integral part of our healthcare team, the LPN will provide high-quality nursing services, support patient assessments, and ensure the delivery of safe, effective care tailored to individual needs. Shift: Part-Time and PRN Opportunities Available
Duties
- Perform comprehensive patient assessments, including vital signs, physical examinations, and health history documentation using electronic health record (EHR) systems such as Point Click Care (PCC).
- Administer medications and injections accurately, including IV infusions and medication management in compliance with HIPAA regulations.
- Assist with complex procedures such as catheterization, tube feeding, wound suturing, and sterile processing while maintaining aseptic technique.
- Provide specialized care for individuals with dementia or Alzheimer's disease, including memory care strategies and behavioral health support.
- Support emergency response efforts by managing airway management, ventilator use, and acute pain management in urgent or trauma situations.
- Collaborate with multidisciplinary teams to develop and implement personalized care plans, discharge planning, and case management strategies.
- Document all patient interactions thoroughly in EMR/EHR systems to ensure accurate medical records.
Qualifications
- Valid LPN license with current state licensure required; additional certifications such as Basic Life Support (BLS) preferred.
- Proven experience working in diverse healthcare settings including long-term care, hospital medicine, emergency room, ICU, or trauma centers.
- Experience providing specialized care such as hospice, palliative care, geriatrics, dementia or Alzheimer’s care is highly desirable.
- Strong assessment skills with the ability to recognize critical changes in patient condition promptly.
- Knowledge of infection control practices and sterile techniques essential for maintaining patient safety.
- Ability to work effectively with individuals with disabilities or developmental delays across various age groups including pediatric and neonatal populations.
- Excellent communication skills combined with a compassionate approach to patient-centered care; ability to work collaboratively within a multidisciplinary team environment. Pay: $22.10 - $24.51 per hour
