This Physical Therapist role offers a Monday–Friday schedule (8 AM–5 PM) with light on-call coverage (1 weekend every 2 months).

What You’ll Do as a Physical Therapist:
As a Physical Therapist, you’ll provide compassionate home-based therapy care to adult and geriatric patients, helping them improve mobility, reduce pain, restore function, and remain safely at home. The Physical Therapist will complete evaluations, develop individualized care plans, perform OASIS assessments, educate patients and families, and collaborate with physicians and interdisciplinary care teams to support recovery and reduce rehospitalizations.
What We’re Looking For:
- Licensed Physical Therapist in the state of Ohio
- Minimum 1 year of Physical Therapist experience in an acute care setting
- Home health experience preferred
- Strong communication, clinical judgment, and documentation skills
- Valid driver’s license and reliable transportation
