Responsibilities
- Conduct thorough patient assessments, including physical examinations and vital signs, to determine health status and identify medical needs.
- Develop and implement individualized treatment plans, including medication management and consulting the appropriate physician team.
- Perform diagnostic evaluations utilizing EMR systems like Epic or Cerner; order and review laboratory tests and imaging studies like chest xray.
- Document all patient interactions accurately in medical records adhering to HIPAA regulations; ensure compliance with CPT coding and ICD-10 standards for billing purposes.
Skills
- Must have at least 6 months of experience in post acute setting- Skilled nursing facility or inpatient rehab
- Proven experience working with diverse patient populations.
- Proficiency in EMR/EHR systems such as Epic, Cerner, or eClinicalWorks.
