Must have MDS experience and knowledge of Point Click Care (PCC).
What you will do:
- Assessment, coordination and completion of resident risk assessments and baseline care plan
- Coordination and timely completion of required MDS supportive documentation
- Management, coordination and accurate completion of MDS assessment(s) within the regulated schedule and time constraits
- Management, coordination and completion of resident care plan staffing process within the regulated schedule and time constraits
- Participate in IDT activities or meetings to include but are not limited to: care plan staffing, quality of care and discharge planning
- Attend morning meeting and quarterly QAPI meetings
- A current, valid nursing license is required
- Minimum of one (1) year experience in long term care, prior MDS experience is preferred.
