Coordinate and manage schedules for patient visits, ensuring proper team assignments
Record and communicate schedules with staff and clinical team members

What You Bring:
High school diploma, or equivalent.
Two (2) years of work experience in home health care or related health program preferred.
Excellent communication and organization skills.
Ability to multitask, ensuring the completion of several tasks, projects, and reports throughout the day.
Intermediate functional knowledge of computer/technology skills including, but not limited to MS Office, MS Excel, navigation of company website, and electronic medical record.
