Duties
- Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
- Monitor patient health by observing vital signs such as blood pressure, pulse, respiration, and temperature
- Support medication administration in accordance with medical instructions and home healthcare regulations
- Document patient care activities accurately using Electronic Health Record (EHR) systems and maintain medical records securely
- Provide specialized care for patients with dementia, Alzheimer’s disease, or other memory impairments; including memory care techniques and behavioral management
- Assist with catheter care, wound care, and other medical procedures under supervision or as trained
- Support patient transfer methods using appropriate equipment such as Hoyer lifts or transfer belts while ensuring safety and comfort
- Offer emotional support and companionship to promote mental well-being and reduce feelings of isolation
- Follow HIPAA guidelines diligently to maintain patient confidentiality at all times
- Support end-of-life care with compassion, dignity, and respect for patient wishes
Requirements
- Proven experience working as a Home Health Aide or in assisted living/nursing home environments; experience with individuals with disabilities or developmental disabilities is highly valued
- Knowledge of medical terminology, vital signs monitoring, medication administration, and basic life support (BLS) including CPR certification
- Familiarity with home health care assistance, hospice care, dementia care, and geriatrics is preferred
- Ability to operate EMR/EHR systems accurately for documentation purposes
- Physical ability to perform heavy lifting, assist with patient transfers using Hoyer lifts or other equipment, and perform cleaning or laundry tasks as needed
- Strong communication skills to effectively interact with patients, families, and healthcare teams
- Understanding of home health regulations and maintaining patient confidentiality according to HIPAA standards
- Valid certification in first aid and CPR; additional certifications in hospice or dementia care are a plus
