Position Overview
We are seeking an experienced APRN who is passionate about caring for patients in a primary care setting and committed to delivering personalized, preventive healthcare.
The ideal candidate enjoys developing meaningful relationships with patients and helping improve overall health outcomes through proactive care management.
Key Responsibilities
- Provide comprehensive primary care services
- Perform patient assessments, examinations, and treatment planning
- Manage acute and chronic medical conditions
- Support preventive care initiatives and wellness programs
- Educate patients and families regarding treatment plans and health management
- Monitor quality metrics and support value-based care initiatives
- Maintain timely and accurate medical documentation
- Collaborate with physicians and interdisciplinary care teams
Candidate Profile
We are looking for someone who is:

- Comfortable managing chronic disease populations
- Experienced in value-based healthcare models

Qualifications
- Active and unrestricted APRN license in Texas (or ability to obtain)
- DEA license or ability to obtain within established onboarding timeline
- Minimum 2–3 years of experience in primary care or value-based care settings
- Experience working with Medicare Advantage, Medicare, and/or Medicaid populations preferred
- Strong understanding of:
- Chronic disease management
- Preventive care
- Quality metrics
- Population health initiatives
- Strong documentation skills
- EMR experience required
- eClinicalWorks (eCW) experience preferred

Job Type: Full-time
Pay: $120,000.00 - $140,000.00 per year
