Queens Center for Rehabilitation & Nursing is seeking a compassionate and dedicated Social Worker to join our interdisciplinary team. This is an excellent opportunity for a caring professional who is passionate about supporting residents and their families while helping them navigate their healthcare journey in a skilled nursing and rehabilitation setting.
As a Social Worker, you will play a vital role in promoting residents' emotional well-being, coordinating services, and ensuring a positive experience throughout their stay.
Complete psychosocial assessments for new admissions and ongoing resident evaluations.
Develop and implement individualized care plans in collaboration with the interdisciplinary team.
Provide emotional support, counseling, and crisis intervention for residents and their families.
Coordinate discharge planning to ensure safe and successful transitions.
Assist residents and families with community resources, financial assistance, and advance care planning.
Advocate for resident rights and promote dignity, independence, and quality of life.
Participate in interdisciplinary care plan meetings and family conferences.
Maintain accurate documentation in accordance with CMS, state, and facility requirements.
Collaborate with nursing, rehabilitation, admissions, and medical staff to support resident-centered care.
Assist with resident adjustment, conflict resolution, and grievance management.
Support regulatory compliance and survey readiness.
Bachelor's degree in Social Work or a related Human Services field required.
Social Work experience in a Skilled Nursing Facility, Rehabilitation Center, Long-Term Care, or healthcare setting preferred.
Knowledge of discharge planning, resident rights, and CMS regulations preferred.
Excellent communication, interpersonal, and organizational skills.
Ability to work collaboratively within a multidisciplinary team.
