Duties
- Assist patients with activities of daily living (ADLs) such as bathing, dressing, grooming, and mobility support using patient transfer methods and equipment like Hoyer lifts.
- Monitor patient health by observing vital signs, reporting changes, and maintaining accurate medical documentation within EMR (Electronic Medical Record) systems.
- Provide medication reminders and assist with medication administration following home healthcare regulations and safety protocols.
- Support patients with cognitive impairments such as dementia or Alzheimer's disease through specialized memory care techniques and compassionate engagement.
- Perform light housekeeping tasks including cleaning, laundry, meal preparation, and ensuring a safe environment aligned with client care plans.
- Document patient interactions thoroughly while maintaining strict adherence to HIPAA regulations to protect patient confidentiality.
- Communicate effectively with clients, family members, and healthcare providers to coordinate care plans and ensure seamless service delivery.
Qualifications
- Proven experience working with individuals with disabilities, developmental disabilities, or in geriatrics settings such as nursing homes or assisted living communities.
- Knowledge of medical terminology, medical records management, and home health care assistance procedures.
- Certification or training in first aid, CPR (Cardiopulmonary Resuscitation), basic life support (BLS), and hospice or end-of-life care preferred.
- Ability to work independently and demonstrate excellent patient care skills with empathy and professionalism.
- Familiarity with EMR/EHR (Electronic Health/Medical Records) systems for documentation purposes.
- Physical ability to perform heavy lifting, assist with patient transfers using methods like Hoyer lifts or patient transfer techniques.
- Strong communication skills for effective client interaction and team collaboration while maintaining patient confidentiality under HIPAA guidelines.
