Registered Nurse (RN) – Home Health Care (Contingent)
We are seeking a compassionate and skilled Registered Nurse (RN) to join our Home Health team on a contingent basis. In this role, you will provide high-quality, patient-centered care in accordance with individualized care plans, established nursing standards, and agency policies. You will collaborate closely with patients, families, and an interdisciplinary team to ensure optimal outcomes.

Key Responsibilities
- Perform initial and ongoing comprehensive patient assessments using the nursing process
- Develop, implement, and update individualized plans of care
- Complete OASIS documentation at required time points
- Administer and document treatments as ordered by the physician
- Deliver skilled nursing care requiring specialized clinical knowledge
- Educate patients and families on medications, disease processes, and self-care techniques
- Assess caregiver capabilities and provide ongoing evaluation of patient and family responses to care
- Communicate patient status and changes to the Case Manager and interdisciplinary team
- Notify physicians of changes in condition and update care plans accordingly
- Participate in discharge planning
- Conduct chart reviews and maintain accurate, timely documentation
- Attend interdisciplinary team (IDT) meetings and case reviews as required
- Participate in Continuous Quality Improvement (CQI) initiatives
- Participate in on-call rotation
Additional Responsibilities
- Attend required agency meetings, trainings, and in-service programs
- Adhere to all organizational policies, procedures, and regulatory requirements
- Maintain strict confidentiality of patient and agency information

Qualifications
Education & Experience
- Graduate of an accredited school of professional nursing
- Minimum of one (1) year of current Home Health care
Licensure & Certifications
- Active, unencumbered RN license in the State of Michigan
- Current BLS/CPR certification
- Valid driver’s license, reliable transportation, and auto insurance
Skills & Abilities
- Strong clinical assessment and critical thinking skills
- Effective communication and patient education abilities
- Ability to work independently and as part of an interdisciplinary team
- Basic math skills (dosage calculations, rates, and percentages)
- Strong documentation and organizational skills
- Commitment to maintaining confidentiality and professionalism
Work Environment & Requirements
- Ability to travel to patient homes
- Participation in an on-call rotation is required
