Job Summary
We are seeking a passionate and dedicated Clinical Dietitian to join our healthcare team, committed to improving patient outcomes through personalized nutritional care. In this vital role, you will assess patients’ nutritional needs, develop tailored care plans, and provide expert guidance on medical nutrition therapy. Responsibilities
- Conduct thorough patient assessments, including nutritional analysis and diagnostic evaluations to determine individual needs.
- Develop and implement personalized care plans that incorporate medical nutrition therapy (MNT), tube feeding protocols, and weight management strategies.
- Provide evidence-based nutrition counseling and health coaching to motivate patients toward healthier behaviors and improved chronic disease management.
- Collaborate effectively with physicians, nurses, dietitians, and other healthcare professionals to coordinate patient care plans.
- Manage electronic health records (EHR/EHR systems) accurately to document assessments, interventions, progress notes, and care outcomes.
- Stay current with advances in clinical nutrition, physiology knowledge, and best practices for acute care, geriatrics, and long-term care settings.

- United States Citizenship:

- Registration:
All applicants must hold the RDN credential from the Commission on Dietetic Registration (CDR) the credentialing agency of the Academy of Nutrition and Dietetics.

Pay: $50,460.00 - $97,087.00 per year
