Position: Lead Care Manager
Location: Office-based (Burbank, CA) with occasional local travel
Reports To: LCM Supervisor
Job Overview
The Lead Care Manager plays a key role in coordinating care for members, helping them set and achieve realistic health and wellness goals. This position requires strong organization, consistent follow-through, and the ability to think critically in a changing healthcare landscape.
You’ll work directly with members, healthcare providers, and community partners to develop and implement individualized Care Plans, ensure documentation accuracy, and navigate complex situations with sound judgment and efficiency.
Key Responsibilities
- Engage members in person or in the community to explain services and build trusting relationships.
- Conduct detailed assessments and develop individualized Care Plans based on each member’s physical, behavioral, and social needs.
- Track progress, monitor outcomes, and update Care Plans regularly to reflect changes in condition or goals.
- Identify barriers, analyze options, and act decisively to ensure members receive the support they need.
- Educate members and their caregivers on self-management and healthier lifestyle choices.
- Communicate effectively with physicians, hospitals, and community providers to align care.
- Connect members to housing, food, transportation, and other essential resources.
- Assist with discharge planning and post-hospital follow-ups to reduce readmissions.
- Maintain thorough, accurate, and timely documentation in the Electronic Health Record (EHR). Attention to detail is critical.
- Adjust quickly to changes in processes, systems, or member needs; healthcare is evolving, and so are we.

Qualifications:
Education: High School Diploma, College Degree Preferred
Skills:
- Strong computer proficiency (Microsoft Office, data entry, EHR systems)
- Excellent written and verbal communication
- High attention to detail and organization
- Reliable attendance and accountability are non-negotiable
Licensing: Valid driver’s license; local travel required.
Language: Fluency in English and Spanish or English and Armenian required.

Job Type: Full-time
Pay: $21.00 - $26.00 per hour
Expected hours: 40 per week
